Checking your Insurance coverage

Kate Farms formulas may be covered by insurance for oral use and tube feeding. Coverage depends on the patient's diagnosis and insurance plan. To check your coverage, follow the steps below. For assistance, contact our Medical Customer Care team at insurance@katefarms.com.

STEP 1: GET A PRESCRIPTION FROM YOUR DOCTOR.
To help facilitate the prescription, it may be helpful to bring your a doctor a Physician's Written Order form. Download a copy on our Resources page.

STEP 2: DOCTOR SENDS PRESCRIPTION TO A SUPPLIER 
Have your doctor's office fax your prescription to a medical supplier. The supplier will contact your insurance company and check your coverage eligibility. If you would like assistance finding a medical supplier, ask your doctor to fax your Physician's Written Order form to us at 805-832-6119 and we will help you find one. STEP 3: BEGIN ORDERING FROM SUPPLIER DIRECTLY
If you are eligible for insurance coverage, the supplier will contact you to go over your options, and set you up with a monthly order to be shipped directly to your home. If you are denied coverage for any reason, please see our Patient Assistance Program application below.
