Insurance Coverage

Kate Farms formulas can be covered by many insurance plans depending on the patient's need for liquid nutrition and insurance eligibility. To check and see if you are eligible for insurance coverage, head through the following:


STEP 1: GET A PRESCRIPTION FROM YOUR DOCTOR.

To help facilitate the prescriptions, you can bring your doctor a Physician's Written Order form. Check out our Resources page for downloadable Physician's Written Order forms.

STEP 2: DOCTOR SENDS PRESCRIPTION TO A SUPPLIER

Have your doctor's office fax your prescription to a medical supplier. The supplier will check your insurance coverage and see if you are eligible for coverage.

IF YOU ALREADY HAVE A MEDICAL SUPPLIER through which you order liquid nutrition or other medical supplies, have the prescription sent to them. If they have trouble ordering, they can contact Kate Farms directly at insurance@katefarms.com or 805.845.2446.

STEP 3: BEGIN ORDERING FROM SUPPLIER DIRECTLY

If you are eligible for insurance coverage, the supplier will contact you to go over your options, and set you up with a monthly order to be shipped directly to you.
